Option 1) Print it out and complete by hand, take a clear photo of each page and pop the images in an email to me. (bring the original when you come in)

Option 2) Download the Microsoft Docx file and edit it directly in Microsoft Word on your PC and email the document to me.​

Option 3) Download the PDF version and edit it using a Mark Up feature on your phone or other app, and email it to me (this option is the most difficult and fiddly).

Option 4) Ask me to post a paper copy out to you. You can either post it back, or send me the photos  once comple​ted.
